This has\n been my best source of identifying failures. This is hard to find, and \nsuper valuable once you do.<\/li><li><strong>Establish success + failure criteria with your direct report(s).<\/strong>\n What are the objective alarms, or guardrails? This helps make it clear \nto identify the failure \u2013 and prevent it from happening in the first \nplace. Also decide when is a good time to evaluate and circle back on \nthis criteria.<\/li><li><strong>Create a forum.<\/strong>\n Sometimes, simply asking \u201cwhat\u2019s not working\u201d and seeking out negative \nfeedback can help you identify failures as you go. However, keep in mind\n that when someone calls out a failure, how you react as a leader is \ncritical. This can make or break the usefulness of the forum. Also, one \nWatercooler member noted how retrospectives are a good way to do this as\n a team \u2013 but only useful for identifying failures in things where you \nhave a specific delivery schedule and are prompted to set a retro up in \nthe first place.<\/li><\/ul>\n\n\n\n<p><em>How to address failure with your team:<\/em><\/p>\n\n\n\n<ul><li><strong>Closure only comes with 100% honesty.<\/strong>\n Everyone knows the elephant in the room. If a project failed, or you \nmessed up, or someone else messed up \u2013 everyone likely already knows it.\n It does no one any good to skirt the issue. Downplaying a failure can \nbe worse than ignoring it.<\/li><li><strong>Eat the elephant one bite at a time.<\/strong>\n The more difficult a conversation and the more complex the issue, the \nmore conversations must exist to uncover the whole topic and to uncover \nthe whole situation.<\/li><li><strong>Leverage one-on-ones.<\/strong>\n The fact that you (hopefully) have a conversation scheduled to talk \nabout what\u2019s on people\u2019s mind gives you the perfect opportunity to talk \nabout the failure, why it happened, and what can be improved. Use this \ntime not as a status update, but to have you sharing thoughts, ideas, \nfears, failures.<\/li><li><strong>Look to foster a \u201cno guilt\u201d culture.<\/strong>\n If you want people to honestly talk about why, how and when they \nfailed, it is necessary to have a safe environment for people to assume \ntheir mistakes. Reveal your own mistakes. Don\u2019t be accusatory.
